Instructions On How to Obtain an Official Transcript

A written request is required according to the Privacy Act Law. Please include the following information.

• Name while in school
• Attendance dates
• Program name
• Indicate graduate or non-graduate
• Social security number
• Address to send transcript
• $5 processing fee (check or money order) payable to the Utah Career College
• Signature authorizing release of grades
